For each request, a fee of 1,100 yen including tax must be transferred to the designated bank account.
Please note that printing and photocopying costs, costs for obtaining identification documents, postage costs, and other expenses required for requests for disclosure, etc., shall all be borne by the individual. Please note that we will not be able to disclose the information or notify the purpose of use if the fee is insufficient.

(6) Purpose of use of personal information acquired through requests for disclosure, etc.
Personal information obtained in connection with a request for disclosure, etc. shall be handled only to the extent necessary for the request for disclosure, etc. Documents submitted will be retained for one year from the date of completion of the response to the request for disclosure, etc., and will then be destroyed.

- The subject of the request for disclosure does not fall within the scope of retained personal data or records provided to a third party as defined in Article 16 of the Act.
- If there is a risk of harm to the life, body, property, or other rights or interests of the person concerned or a third party
- If there is a risk of significant hindrance to the proper conduct of the hospital's business
- If the Company's business is in violation of any other law or regulation.
